Summary
The facility failed to revise the care plan for a resident after the individual qualified for Preadmission Screening and Resident Review (PASRR) Level II services. Record review showed that the resident's PASRR, dated 4/2/24, indicated a referral for a Level II face-to-face onsite assessment, but the document was not initially found. The Director of Nursing later provided the PASRR Level II document, dated 4/15/24.
