Summary
The facility failed to complete annual performance evaluations for five sampled Certified Nursing Assistants (CNAs) who were all hired on the same date. Documentation confirming the completion of annual performance evaluations for the year following their hire was not available for any of these employees. During an interview, the Administrator and Regional Director of Operations explained that when the previous administration sold the company, all employee records were taken, and all staff were considered new hires as of the same date.
