Summary
Surveyors observed that the facility failed to maintain a sanitary environment for respiratory care equipment for two residents. For one resident, a nebulizer mask and tubing were repeatedly found unbagged and left on the bedside table, despite an active physician order for nebulizer treatments as needed.
