Specifically, for five out of seven residents reviewed, there was no evidence that an IDT meeting occurred within the required timeframe after the most recent MDS assessments. The clinical records for these residents showed that the last IDT meetings were held outside the 7-day window or were missing entirely following the assessments. Interviews with facility staff, including the Social Service Assistant and the Area Manager of Clinical Reimbursement, confirmed that the required IDT meetings did not occur within the mandated period after MDS completion. The deficiency was identified through both record review and staff interviews, which substantiated that the care planning process was not conducted in accordance with regulatory requirements for timely interdisciplinary review and revision.
