Summary
The facility failed to ensure that the care plan was reviewed and revised by an interdisciplinary team (IDT) with the participation of the resident, as required. One resident reported never having heard of care plan meetings, and a review of the medical record showed that while IDT meetings were held on several occasions, there was no evidence that the resident was invited to or participated in these meetings.
