Summary
The facility failed to provide mandatory Quality Assurance and Performance Improvement (QAPI) training to all direct care staff as required. Record reviews showed that five sampled Certified Nursing Assistants (CNAs), each with documented hire dates in 2025, did not receive QAPI training. Interviews with the CNA Supervisor revealed that QAPI training was not included in either new hire orientation or ongoing in-service education.
