This lack of documentation was consistent across all reviewed cases, regardless of the admission or re-entry date. During an interview, the Business Office Manager confirmed that the medical records for these residents did not contain evidence that the required information about advance directives had been provided. The deficiency was identified through both record review and staff interview, with no indication in the records that the residents or their representatives were informed of their rights concerning advance directives.
