Summary

The facility failed to provide a Baseline Care Plan summary to two residents and their representatives within 48 hours of admission, as required. For one resident with severe cognitive impairment, the Baseline Care Plan was completed on the day of admission, but there was no documentation that the summary was provided to either the resident or their representative. The signature and date boxes for both the resident and representative were left unsigned and undated, and the facility could not produce any evidence that the summary had been given. The resident's representative confirmed in a telephone interview that they had not received the Baseline Care Plan summary. Similarly, for another resident who was cognitively intact, the Baseline Care Plan was completed on the admission date, but again, there was no documentation that the summary was provided to the resident or their representative.
