Summary

A resident with diagnoses of dementia and weakness, who was assessed as having moderately impaired cognition and being dependent on staff for all activities of daily living (ADLs), did not receive appropriate nail care. The resident's care plan and assessments indicated a need for staff assistance with personal hygiene, including nail care. Despite these documented needs, observations over two consecutive days showed that the resident's fingernails were long, jagged, and dirty. Interviews with certified nurse aides confirmed that the resident was dependent on staff for all ADLs, including fingernail care, and that nail care was typically performed on shower days.
