Summary

The facility failed to ensure that the posted daily nurse staffing sheets included accurate and identifiable information, specifically the actual hours worked by licensed and unlicensed staff. Observations on multiple dates revealed that the posted staffing sheets did not list the actual number of hours worked, and this issue was found on several days throughout the month.
