During the review period, the facility reported a census of 21 residents but did not accurately report licensed nurse coverage for multiple dates, as identified in the PBJ Staffing Data Report. Specifically, the Unit Coordinator/Registered Nurse (RN) covered nursing shifts on several dates, but these hours were not reported to PBJ. Additionally, an RN who typically worked in another area covered a nursing shift, but her hours were not coded correctly and thus not reported. The Director of Nursing (DON) confirmed that these nursing hours were omitted from the PBJ submission and acknowledged that the facility did not have a PBJ policy in place.
