Summary
The facility failed to complete a Discharge Minimum Data Set (MDS) and a Reentry MDS for a resident who was hospitalized and subsequently returned to the facility. Record review showed that the resident was discharged to the hospital and then readmitted, but the required Discharge and Reentry MDS assessments were not opened, started, or completed as mandated. Staff interviews confirmed that the assessments were not initially flagged for the MDS Coordinator, and the coordinator was not onsite at the time.
