Summary

The facility failed to provide food portions as directed by the dietary spreadsheet approved by the registered dietician for four residents out of a sample of seventeen reviewed for dining. Specifically, residents with complex medical histories, including diabetes, chronic kidney disease, dementia, malnutrition, and other conditions, were served ham portions that were less than the required 3 ounces specified on the dietary spreadsheet. The dietary manager weighed the ham and found it to be just under 2.75 ounces, and this amount was served to multiple residents.
