Summary

The facility failed to follow its policy and procedure for timely reporting of an allegation of resident-to-resident abuse. On 4/27/25, a resident with chronic kidney disease and no cognitive deficits alleged that another resident, who has schizoaffective disorder and mild cognitive impairment, engaged in inappropriate behavior toward him. The incident was documented in progress notes, and immediate actions were taken, including separating the residents, initiating 1:1 monitoring, conducting a body check, and notifying the medical doctor, family, and police. A petition for involuntary admission was also completed for the alleged perpetrator.
