The facility failed to provide skin treatments as ordered by the physician for two residents who were being monitored for skin alterations. The medication administration records indicated that blood glucose testing was not performed as ordered before meals and at bedtime on two specific occasions. The Director of Nursing confirmed these omissions during record review. These failures represent non-compliance with the facility's own policies for wound care and blood glucose monitoring.
