The facility failed to provide meals in the appropriate form for four residents who were prescribed pureed diets. However, during meal preparation, the cook pureed breaded fish sticks and barbecue pork, adding warm milk, but the resulting mixtures were observed to be very thick and not smooth. When served, the pureed foods were found to be thick, dry, and lacking the required smooth consistency, which did not meet the dietary needs of the residents on pureed diets. The dietary manager confirmed that the pureed foods should have been smooth and homogenous, without lumps or chunks.
