Summary
The facility failed to provide mandatory annual in-service training on the Quality Assurance and Performance Improvement (QAPI) program to all staff, as required by its own policy. Record review showed that the annual in-servicing calendar specified QAPI training for all staff in March, but verification by the Director of Nursing confirmed that no staff had received this training for the current year. At the time of the survey, 79 residents were documented as residing in the facility, and this lack of training had the potential to affect all of them.
