Summary

The facility failed to assess, monitor, and identify potential triggers for a resident diagnosed with Post-Traumatic Stress Disorder (PTSD), as required for trauma-informed care. Record review showed that the resident had multiple diagnoses, including PTSD, paraplegia, anxiety, and major depressive disorder, and was taking antianxiety and antidepressant medications daily.
