Summary
Facility A failed to ensure proper disposal and containment of garbage and refuse in one of its buildings, specifically building B. During an observation behind the kitchen, surveyors found two dumpsters with their lids and side compartment doors open, trash including old chairs behind the dumpsters, a trash can filled with unbroken boxes, and six empty 35-gallon plastic oil jugs lying on the ground instead of being disposed of in the dumpsters.
