Summary

The facility failed to ensure timely notification of changes in condition to medical providers and family members for two residents, resulting in death for one and actual harm for another. For one resident with multiple diagnoses including congestive heart failure, diabetes, and deep vein thrombosis, there were repeated instances where significant changes such as worsening edema, bruising, and absent pedal pulses were documented in the medical record, but not communicated to the resident's medical provider.
