Summary

The facility failed to complete and update the Pre-admission Screening and Resident Reviews (PASARRs) for two residents with qualifying mental health diagnoses. For one resident admitted with primary and secondary diagnoses including dementia, mood disorder, depression, irritability, anger, bipolar disorder, anxiety, and failure to thrive, the Level I PASARR was found to be incomplete, with all qualifying diagnoses not checked and no Level II PASARR submitted for further consideration.
