Summary

The facility failed to forward a resident for a level 2 Preadmission Screening and Resident Review (PASARR) as required. Record review for a resident revealed that the PASARR form, dated 7/16/20, indicated both depression and dementia, and the completion section specified that a level 2 screening was needed due to suspicion of serious mental illness.
