The facility failed to ensure that two residents were offered the COVID-19 vaccine as required by facility policy. Both residents were admitted to the facility and subsequently sent to the Emergency Department, where they tested positive for COVID-19 before being re-admitted. During an interview, the Infection Preventionist confirmed that the residents were not offered the vaccine upon admission, which is a standard part of the facility's admission process.
