Summary

The facility failed to maintain an effective pest control program, resulting in the presence of roaches within the building. During observations, a roach was seen crawling on the wall behind a resident who was in bed, and another roach was observed outside a resident's room in the 300 South Wing. Additionally, while interviewing a resident's wife, a roach was seen crawling up the wall in the same wing. The resident's wife also reported the presence of bugs in the room. The Administrator confirmed that pest control services are provided weekly and acknowledged recent sightings of pests, including lizards and baby lizards, both inside and outside the facility.
