Summary

The facility failed to provide necessary assistance with activities of daily living (ADLs) for two residents. One resident, admitted with multiple diagnoses including neuromuscular dysfunction and multiple sclerosis, was observed with long white facial hair and expressed a desire for help with its removal. Her records indicated she required supervision or assistance for oral hygiene and was dependent on staff for bathing.
