Summary
Facility staff failed to maintain an environment free of pests, as evidenced by the presence of multiple mouse traps and mouse droppings in key food preparation and cleaning areas. Specifically, three mouse traps were observed around the cook line, including behind the grill and gas stove, and another mouse trap with mouse droppings was found in a corner of the dishwashing machine room. Additionally, flying insects, identified as gnats, were observed in the area of the three-compartment sink and sporadically throughout the kitchen.
