The facility failed to provide adequate supervision to prevent sexual abuse between two residents, both of whom had diagnoses of dementia and other cognitive impairments. The care plans for both residents acknowledged a consensual, friendly relationship that was approved by family members, with specific interventions in place such as allowing hand-holding, hugging, and kissing, but not sexual acts.
