Summary

The facility failed to ensure that clinical records were complete and accurate for three residents reviewed for quality of care. For one resident with dementia and a history of pressure ulcers, physician orders required specific wound care interventions to be performed and documented every day and evening shift. However, review of the Treatment Administration Record (TAR) revealed that wound care was not documented on several occasions across multiple months. Interviews with LPNs responsible for the care confirmed that while the care was reportedly provided, documentation was omitted due to forgetfulness. The Director of Nursing (DON) stated that nursing staff are expected to document care accurately and timely in the electronic medical record, as outlined in the facility's documentation policy. For another resident with heart failure and an open wound, physician orders directed specific wound care and medication administration.
