Summary

A deficiency occurred when a resident with a history of malignant neoplasm of the frontal lobe, epilepsy, anxiety disorder, and PTSD was not protected from verbal and emotional abuse. The abuse policy required immediate reporting and investigation of all abuse allegations, but these procedures were not followed. The LPN was unaware of the care plan requirement for two caregivers, and staff failed to ensure the resident's safety and dignity as outlined in facility policy.
