During interviews, the Administrator was unaware of the issue and deferred responsibility to the Director of Nursing Services (DNS), who stated that another staff member was responsible for PBJ data entry but could not explain the discrepancy. The DNS later provided staff schedules confirming that licensed nursing staff were present on all shifts for the days in question. Attempts to interview the staff member responsible for PBJ data entry were unsuccessful. Facility documentation and interviews confirmed that the required staffing information was not accurately reported, despite the presence of licensed staff.
