Summary

The facility failed to maintain a safe and operable environment for residents by not ensuring that an electrical outlet in one of the resident rooms was in good repair. During an observation, it was found that the wall electrical outlet outside the bathroom in the specified room was missing a cover plate, leaving electrical wiring exposed.
