Summary

The facility failed to ensure that three shower rooms were properly cleaned and disinfected, specifically neglecting to clean the shower heads. During an interview, a resident reported that the showers appeared to have fecal matter present. Observations confirmed the presence of dark brown residue on the lower half of the shower heads and brown stains on the walls and shower head holders in the shower stalls.
