Summary

The facility failed to report an allegation of physical abuse to the California Department of Public Health (CDPH) within the required two-hour timeframe after the allegation was made. A resident with moderate cognitive impairment and a history of osteomyelitis reported an altercation with his roommate, during which he claimed his arm was twisted and he was scratched, resulting in visible scabbed marks above his wrist. The incident was documented in a social service progress note, and the resident expressed feeling unsafe that night.
