Summary

A deficiency occurred when a resident with diagnoses including type 2 diabetes mellitus, schizophrenia, and adult failure to thrive was not kept clean, as observed on 8/5/2025. The resident was found lying in bed with an oily, unkempt ponytail and the bottoms of both bare feet covered with black dirt. Multiple staff, including a CNA and an LVN, confirmed the resident's disheveled appearance and dirty feet during interviews and observations. The resident's care plan indicated a need for assistance with activities of daily living (ADLs), including personal hygiene and bathing, and interventions were in place to encourage participation and maintain cleanliness.
