Similarly, another resident with unspecified dementia, muscle wasting, and cognitive communication deficits also had a care plan that did not identify specific behaviors to monitor. Staff interviews and record reviews confirmed the absence of individualized behavioral interventions in the care plan. The facility's policy required comprehensive, person-centered care plans to be updated as residents' conditions changed, but this was not followed for these residents, resulting in care plans that were not tailored to their current needs and behaviors.
