Summary

The facility failed to provide written transfer/discharge notices at least 30 days before the transfer for three residents who were moved from Polaris Extended Care to Polaris Transitional Care. The record showed that each resident had significant medical needs, including chronic respiratory failure and tracheostomy care, and each was non-communicative. For two residents, the transfer was discussed with the guardian or health care agent in March 2025, but the physician orders authorizing the move were not entered until June 2025. For the third resident, the physician order for transfer was dated in April 2025 after the move had already been discussed with the guardian.
