The DON and administrator both stated that care plans should be updated in real time and reflect the resident's current condition, but the MDS nurse was unsure of her role in the process. The facility's policy requires ongoing assessment and updating of care plans when a resident's clinical status or condition changes, but this was not consistently followed for the residents reviewed.
