Summary

Surveyors identified that the facility failed to provide thickened liquids as ordered for three residents with dysphagia and a history of aspiration risk. Medical record reviews for these residents showed clear physician and care plan orders for specific thickened liquid consistencies, such as level 4 extremely thick or level 3 moderately thick liquids, due to their impaired swallowing abilities and high risk for aspiration and pneumonia. Despite these orders, observations revealed that unthickened water pitchers were present at the bedsides of these residents. During observations, staff confirmed that the water provided to these residents was not thickened according to their dietary requirements.
