The facility failed to develop a baseline care plan with specific goals and interventions to address the immediate needs of a newly admitted resident within 48 hours of admission. The resident, who had diagnoses including diabetes and dementia and was able to communicate needs, was assessed as having impaired vision and did not have corrective lenses available at the facility. Despite the resident expressing a desire to have their eyeglasses brought from home, the care plan did not include any interventions or plans to obtain the eyeglasses or arrange for new ones.
