Summary

The provider failed to assess and address the trauma-related needs of two residents diagnosed with post-traumatic stress disorder (PTSD). Both residents had moderate cognitive impairment and complex psychiatric histories, including anxiety, depression, hallucinations, and a history of chemical dependency.
