Summary

The facility failed to prevent neglect of a resident by not following established safe resident care guidelines, resulting in significant harm. According to the facility's Safe Resident Handling Program and the resident's care plan, the resident required two-person assistance for all care and extensive or total assistance for turning and repositioning in bed. Despite these documented requirements, a nurse aide provided care to the resident alone, without the required assistance, and allowed the resident to roll over independently during care. This action was contrary to the resident's care plan and the facility's policies, which were designed to prevent such incidents. The resident involved had multiple complex medical conditions, including heart failure, respiratory failure, renal failure, diabetes, morbid obesity, and lymphedema, and was assessed as being at risk for falls due to impaired mobility.
