Summary
The facility failed to provide mandatory training on its Quality Assurance and Performance Improvement (QAPI) program to four out of five reviewed employees, including three nurse aides and one LPN. According to the facility assessment, staff training on QAPI is required during general orientation upon hire, annually, and as needed. Document review showed that Nurse Aide Employees E10, E11, and E12, as well as LPN Employee E8, did not receive QAPI in-service education within the specified annual periods.
