Summary

The facility failed to develop a resident-centered care plan for a resident with severe cognitive impairment and a diagnosis of dementia. The resident's Minimum Data Set (MDS) and activity assessments indicated preferences for reading, music, animals, fresh air, and specific types of books, such as murder mystery and suspense.
