Summary

The facility failed to provide a safe, clean, and homelike environment for its residents, as evidenced by multiple environmental hazards and lack of timely corrective action. In the outdoor smoking area, a resident expressed concerns about safety due to the deteriorating condition of the wooden structure, which was observed to have extensive rot, a large hole caused by termite damage, and separating support beams.
