Similarly, another resident with diagnoses such as chronic obstructive pulmonary disease, heart failure, and intact cognition, who was also receiving hospice services, reported not having attended a care conference. The Social Services Director verified that the last care conference for this resident occurred more than a year ago, and no recent formal care conference had been conducted, despite ongoing informal communication with the resident's family.
