Summary

The facility failed to ensure that care plan conferences were held for two residents as required. For one resident with multiple diagnoses including end stage renal disease, diabetes, and mild cognitive impairment, there was no evidence of a recent care conference. The resident stated she did not recall ever having a meeting to discuss her care or treatment goals. The Social Services Director confirmed that the most recent care conference was held over a year prior, and although staff communicated with the resident's family, no formal care conference had taken place.
