Summary
Facility staff failed to update the Preadmission Screening and Resident Review (PASARR) assessment for a resident with multiple diagnoses, including cerebrovascular disease, chronic obstructive pulmonary disease, neurocognitive disorder, and diabetes, after a significant change in condition.
