Summary

The facility failed to ensure it was administered in a manner that enabled effective and efficient use of its resources to attain or maintain the highest practicable well-being of each resident. Specifically, the facility did not have an active Administrator on-site on a full-time basis, as required by its own policy and job description. The Administrator position was filled on an interim basis by an individual who also served as Administrator for two other facilities, resulting in her being physically present at the facility only two to three days per week, according to multiple staff interviews. The Ombudsman also reported receiving complaints from residents and family members about the absence of a full-time Administrator. The interim Administrator herself stated that she worked a 40-hour week but was not present in the facility every day, as she was responsible for two other facilities as well. The lack of a full-time, on-site Administrator led to challenges in leadership, oversight, and compliance with state and federal regulations, as well as difficulties in addressing ongoing survey citations and complaints. The facility census at the time was 123 residents.
