Summary

The facility failed to provide necessary behavioral health care and services when staff did not intervene appropriately during incidents involving agitated and aggressive resident behavior. One resident, who was cognitively intact and had a history of depression and poor impulse control, became agitated and verbally aggressive, escalating to physically kicking another resident who was severely cognitively impaired and had a history of daily physical behavioral symptoms.
