The facility failed to ensure that care conferences were conducted as required for three residents reviewed for care planning. A third resident, also cognitively intact and with a history of amputation and other medical conditions, did not recall having a care conference. Social services staff indicated that care conferences were sometimes not conducted if the resident declined, and the interdisciplinary team did not always meet without the resident to review care areas. The director of nursing clarified that care conferences should occur every 92 days regardless of resident attendance. The facility's policy required care conferences to develop and review care plans with the interdisciplinary team, but this was not consistently followed.
