Summary
The facility failed to document that a resident was offered and/or received a shower on their assigned shower days. The resident, who was admitted for rehabilitation following a lumbar vertebra compression fracture and had intermittent confusion requiring extensive assistance with activities of daily living (ADLs), did not have any record of bathing on two specific days and only received bed baths for a subsequent two-week period.
